Welcome to Collinsville Middle School! We are excited to meet our new students and families and welcome back our returning students and families as we partner with you to make this a great year. Starting the new school year is exciting and filled with opportunities for growth, discovery, and success. At CMS, we prioritize academic excellence and strive to provide an environment where every student can thrive and succeed.

• 7th Grade: An updated Tdap vaccine or approved exemption is required before a schedule can be released. Shot records can be obtained online at https://shotrecords.health.ok.gov/. Guardians interested in obtaining an exemption must do so online through imm-public.osdh.immytech.com. Guardians are responsible for providing the approved exemption information to the school before a schedule can be released.

• Student registration/district enrollment must be completed in full to pick up a schedule. Student registration/district enrollment forms must be updated yearly. https://www.collinsville.k12.ok.us/o/cps/page/enrollment-center

• Students are welcome to walk around the building to become familiar with the location of classrooms. (Please note that teachers will not be on site during this time.)

• Student Arrival: 8:30 a.m. Tardy Bell: 8:45 a.m. Student Dismissal: 3:30 p.m.

• Car riders and walkers will enter and exit the building from the south entrance to help with traffic flow. Center Street is for buses only from 8-9 a.m. and 3-4 p.m.

Reminder- All student athletes must have a physical to participate in athletics.
