May 2025 Finals Blue Slip Form
Please fill out this form if you will be checking out your student to be picked up/leave campus after finals on May 22nd (May 15th for seniors). Please note that if you check out your student, they will need to be picked up, a walker, or a driving student. There will be no buses to deliver students home immediately after testing on this day, but buses will be running as normal before and after school.
**Parents must still complete the form even if your student is exempt
NOTICE: By signing, parents/guardians are giving their student(s) permission to leave campus after completing their Semester Finals.
All medication must be picked up between now and Thursday, May 22nd at 3:30pm. Any medication left in the health office will be destroyed on Friday, the 23rd. Per policy, medication cannot be sent home with the student, it must be picked up by an adult. Please reach out to the school nurse if these times will not work for alternate availability. Thank you for allowing us to help care for your student's health needs!

Parents - Student enrollment for the 2025-2026 has officially opened. Please check our website at the link below for more information. Each year, parents/guardians are required to update and confirm information for all Collinsville Public Schools students. Please complete registration by May 31, 2024.
Collinsville High School will be offering two summer driver’s education classes. The cost to attend either class will be $300.00 and enrollment will be based on a first come-first served basis. Students are qualified to take the class if they are or turn 15 during the session.
The first session will be held in June with the second session being held in July. Please send an email with your desire to attend to Mr. Johnston at the middle school. His email address is pjohnston@collinsville.k12.ok.us

With the end of school, there will not be a Cardinal Math Challenge problem this week, but the CHS Math Department would like to thank all students, family members, and staff who participated in the Cardinal Math Challenge this year!
